Birth to 3 Program

The Birth to 3 Program is Wisconsin's early intervention program for infants and toddlers from birth to age 36 months who present a 25% delay in their development, or have been diagnosed with a long-term disability or medical condition that affects their development.

Children's Long-Term Support (CLTS) Medicaid Waiver

The CLTS waiver is available to children and youth under the age of 22 who have a diagnosis of an intellectual and/or developmental disability, physical disability, and/or mental health disability. To receive services in Dane County, a child must be living in Dane County and be eligible for Medicaid. A child/youth must also complete a Functional Screen to determine his or her level of care.  Dane County services for children with delays or disabilities including the Katie Beckett Program, the Children's Long Term Support Program (CLTS) and the Children's Community Options Program (CCOP) contact the Children's Intake staff at (608) 242-6226 or cltsintake@danecounty.gov

Dane County offers the Vocational Futures Planning and Support (VFPS) service to transition age youth and young adults ages 18 to 21 with Intellectual and Developmental Disabilities (I/DD).
